If your study included patients and/or staff members as subjects as well as resources and time at the VA Salt Lake City Health Care System (VASLCHCS), there are things to consider when submitting applications.
New Studies
Includes the VA Form 10-1223 and a consent using the VA Consent Template with your IRB submission. Be sure to indicate that the VASLCHCS as a location where the study is being conducted.
Renewal Application
Include a consent using the VA Consent Template. Be sure to indicate that the VASLCHCS is a location where the study is being conducted.
Amendment Application:
Include revised consent forms on the VA Consent Template, if applicable. Be sure to answer "YES" when it asked if the study is being approved at the VA or the amendment is requesting that the VA be included as a study site.
Adverse Events:
Include revised consent forms on the VA Consent Template, if applicable. Be sure to answer "YES" when it asked if the study is being approved at the VA.
Final Project Reports:
Be sure to answer "YES" when it asked if the study is being approved at VA.
The IRB will forward copies of any approvals to the VA if you have indicated that the VA is involved. If you do not provide this indication, we cannot guarantee that this will be done.
You are required to obtain approval from the Research and Development (R&D) Office once the IRB has approved any of these applications. Studies cannot be conducted or continued without VA R&D approval. Please contact the VA Research Compliance Officer at 801.582.1565 ext. 4866 for their submission requirements.
