Disclosure Forms
NOTICE: AFTER AUGUST 1, 2005, PAPER DISCLOSURE FORMS WILL NO LONGER BE ACCEPTED BY OSP, IRB OR THE COI OFFICE. PLEASE USE THE ELECTRONIC RESEARCH INTEGRITY AND COMPLIANCE ADMINISTRATION (ERICA) SITE (https://erica.research.utah.edu/erica) FOR SUBMISSION OF COI DISCLOSURE FORMS.
Instructions: New Proposals Being Submitted to OSP
Principal Investigators must submit a Conflict of Interest disclosure form for every new request for funding.
Please use the online Electronic Research Integrity and Compliance
Administration (ERICA) site for conflict of interest disclosures
(see below for link). You will have to use your uNID and associated
password to login, provide some contact information, and then go
to the CoI Disclosures tab and follow the directions. You will get
an email response within 5 minutes of submitting the disclosure.
That response will have your CoI disclosure number and a preliminary
determination of the potential for a conflict of interest. You will
use CoI disclosure number on your OSP Document Summary Sheet (and
your IRB application, if applicable) to confirm that you have submitted
a CoI disclosure.
Instructions: Projects being Submitted to IRB
Principal Investigators must submit a Conflict of Interest disclosure
form for all new projects submitted to the IRB that have not been
previously submitted to OSP.

can go to the COI Disclosures tab and follow the directions. You
will get an email response within 5 minutes of submitting the disclosure.
That response will have your CoI disclosure number and a preliminary
determination of the potential for a conflict of interest. You will
use the CoI disclosure number on your IRB application to confirm
that you have submitted a CoI disclosure.
Instructions: Procurement/Purchasing Disclosures
Disclosures related to purchasing/procurement authority should be filled out and submitted to the Conflict of Interest office.

password to login, provide some contact information, and then you
can go to the COI Disclosures tab and follow the directions. You
will get an email response within 5 minutes of submitting the disclosure.

determination of the potential for a conflict of interest.
